Instructions:
1. Patient obtains a prescription from their doctor.
2. Patient fills out the Medical Massage Forms and makes a "tentative" appointment.
3. I will contact your insurance carrier to confirm massage therapy coverage, what percenage of the bill will the insurance cover, and the patient's co-pay amount. Once confirmation is received the scheduled appointment may take place.
4. The patient is required to pay any co-pay amount due at the time of each session.
5. IMPORTANT: the patient is responsible for any unpaid amount the insurance company does not pay. This means, even after the patient has paid their co-pay amount, if the insurance company does not pay any or part of the remaining amount due, it is the client's responsibility to pay all unpaid amounts.
6. If you need a massage immediately and can't wait for the insurance confirmation, your session(s) must be paid in full at the time of your appointment. After insurance confirmation has been received you may submit a claim to receive reimbursement.
